Pre-Determination Instructions

In order for you to obtain childcare we must gather information to determine your eligibility.

Both You and Your spouse MUST meet one or more of the following: Employment, School (High School or College [NOTE:College hours must not exceed 65 credit hours] Training (Vocational or Technical) A one parent household must have at least 25 hours of activity in a one week period. A two parent household must have at least 50 hours of activity between both parents in a one week period. Income guidelines and other criteria must also be met.

TO PROCESS YOUR PRE-DETERMINATION YOU MUST SUBMIT COPIES OF ALL OF THE FOLLOWING DOCUMENTATION:

♦   Each Parent’s     SOCIAL SECURITY CARD AND IDENTIFICATION CARD
♦   Each Child’s       SOCIAL SECURITY CARD
♦   Each Child’s       BIRTH CERTIFICATE AND IMMUNIZATION RECORD

If Employed:
♦      Four recent check stubs or if new employment, employment verification form or letter form or letter from employer (on letterhead and typed) reflecting income and hours of employment.
♦      Last income tax return filed with the Internal Revenue Service (IRS)
♦     Proof of any other income and/or benefits you receive
♦     Proof of child support income or letter from the Attorney General’s office Form 1825 that a case is open with their office for each child in the  household.

If in School or Training:
♦   Statement of Charges          ♦ Copy of class schedule            
♦   Transcripts of hours or credits accumulated

If attending High School:

♦A letter of Enrollment on letter head and typed from the school you are
   attending.

At the time of your appointment, you MUST have the following:
Daycare:                                                         

♦Name of Daycare                                                       

♦Provider and child must not reside in same residence
                                              

♦Valid Picture Identification Card

♦Make sure daycare has space for your child, visited the facility and  that it is adequate for your needs 

♦Social Security Card    
                

♦Background check from TDPS
             

♦Relative must accompany you to your appointment
